Please provide your Kit Serial ID. You will need this ID to check the status of your kit, make any changes to the information you provide here, and discuss the results of your test(s).

Only the information you provide on this page will be used in processing your samples and generating your report. This is your official Chain of Custody document. As a laboratory, we must abide by the information provided here and will disregard any letter, form, or information included with your kit.

Your report will be issued to you as a PDF attachment via email. Provide at least one email address to receive your report. You will need to know the email address you used to check the status of your kit, make any changes to the information you provided here, and discuss the results of your test(s) with the laboratory.

If you would like additional email address(es) to receive a copy of the report, please include these as well.

You may add up to 10 samples. Each sample must have a unique Sample Number and Sample Location. You may choose to include an Expanded Profile for each sample, and optionally select Rush TAT for expedited processing.

Normal turnaround times are 5-7 days from receipt, not including holidays. Rush analysis is completed by the end of business the next business day after your kit is received. We recommend Rush analysis ($15 per sample) if you must receive results before any particular deadline. Moldlab is open Monday-Friday, 8 AM to 5 PM Central Time. You will need to ship your samples to the laboratory via a method of your choosing. We strongly recommend a service that includes tracking. Do not use stamps to ship your samples. Using stamps may cause your kit to be delayed or returned, or the kit may be delivered to us postage due. For your convenience, we offer optional FedEx shipping labels at a discounted price on this page.
